HOW DO I MAKE AN APPOINTMENT?

The fastest way to schedule an appointment is to fill out the

New Patient Intake Form requesting to establish with a counselor, a medication provider or both. Please indicate your insurance as well. We will reach out to you to schedule an appointment.

WHAT IS YOUR NEW PATIENT PROCEDURE?

Please fill out the New Patient Intake Form. Once connected with one of our providers, we will reach out to you with appointment confirmations and to complete intake paperwork via email. Subsequent appointment times are usually handled in the intake appointment.

DO YOU ACCEPT INSURANCE?

We are contracted with most major insurance companies and we will gladly bill insurance as a courtesy to you. It is your responsibility to know your insurance mental health benefits and co-pay. Please contact us at billing@pnwbh.com if you have any questions!

DO YOU ACCEPT MEDICAID?

We have partnerned with CareOregon/Health Share as a behavioral health coordinated care organization (CCO). If your Medicaid is through CareOregon/Health Share, we are happy to work with you. Please contact us at billing@pnwbh.com if you have any questions!
